Liability Waiver

I, the undersigned parent or legal guardian, acknowledge that participation in soccer activities organized by FC Bosna involves inherent risks of physical injury. These risks include, but are not limited to:

• Sprains, strains, fractures, concussions, and other musculoskeletal injuries
• Injuries resulting from contact with other players, equipment, or field surfaces
• Injuries caused by uneven terrain, weather conditions, or facility hazards
• Heat-related illness, dehydration, or overexertion
• Injuries sustained during travel to and from practices, games, or events

I understand that these risks cannot be entirely eliminated and voluntarily assume all risks associated with my child's participation in FC Bosna activities.

In consideration of my child being permitted to participate in FC Bosna activities, I hereby release, waive, discharge, and covenant not to sue the following parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) that may be sustained during participation:

• FC Bosna, including all officers, directors, and board members
• All coaches, assistant coaches, trainers, and volunteers
• Facility owners, operators, and staff at all training and game locations
• Other players and their parents or guardians
• Any sponsors, affiliates, or partnering organizations

This release applies regardless of whether the injury or loss is caused by negligence, fault, or any other basis of liability of the released parties.

I grant FC Bosna staff, coaches, and authorized volunteers permission to authorize and obtain emergency medical treatment for my child in the event of an injury or medical emergency during club activities if I cannot be reached in a timely manner. I understand that:

• FC Bosna will make reasonable efforts to contact me or my designated emergency contact before seeking treatment
• I am responsible for any and all medical expenses incurred on behalf of my child
• FC Bosna does not carry medical insurance for individual participants; I am responsible for providing adequate insurance coverage for my child
• I agree to inform coaches of any medical conditions, allergies, or medications that may affect my child's participation

I grant FC Bosna permission to photograph and/or video record my child during club activities. I understand that these images and recordings may be used for:

• Club website, social media accounts (including Instagram, Facebook, and TikTok), and newsletters
• Promotional materials, flyers, banners, and advertisements
• Training and development documentation
• Media publications and community outreach

I waive any right to compensation for the use of such images or recordings and understand that these materials may be used indefinitely unless I submit a written request for removal.

I acknowledge that both my child and I (as parent/guardian) are expected to adhere to the FC Bosna Code of Conduct, which includes:

• Treating all players, coaches, referees, and spectators with respect and sportsmanship
• Refraining from abusive, threatening, or unsportsmanlike language or behavior
• Following the instructions of coaches and club officials at all times
• Being punctual for practices, games, and events
• Respecting all facilities, equipment, and property
• Supporting a positive and inclusive team environment

I understand that violation of the Code of Conduct may result in disciplinary action, including suspension or dismissal from the club without refund.

I acknowledge and agree to the following health-related terms:

• My child is physically fit and has been cleared by a physician to participate in soccer activities, or I assume full responsibility for my child's ability to participate
• I will keep my child home from all club activities if they exhibit symptoms of illness (fever, persistent cough, vomiting, etc.)
• I understand that participation in group sports carries a risk of exposure to communicable diseases, including but not limited to COVID-19, influenza, and other contagious conditions
• I will notify FC Bosna immediately if my child tests positive for a communicable disease after attending a club activity
• I agree to follow any health and safety protocols established by FC Bosna, including those related to infectious disease prevention
